Referring Patients to Carle

Physicians can refer most patients to Carle Clinic Association and Carle Foundation Hospital by using the toll-free Physician Access Line (PAL).  

Through the PAL, an RN is available 24/7 to assist with your admission/transfer needs.  You can also initiate this process by faxing the completed Same Day Admit/Transfer Request Form.

Outpatient Appointment Scheduling and Consultations
Our scheduling center specialists will assist with your appointment and consultation needs at Carle Clinic Association. After you or your staff provides the necessary patient information for appointment scheduling, the scheduling center specialist will:
  • Orchestrate the appointment or consultation that you have requested. This can be done while the patient is at your office. Or if you prefer, the scheduling center specialist will call the patient so they may coordinate the appointment with their personal calendar.
  • Confirm appointment details with you and your patient.
  • Provide any additional information necessary to prepare the patient for the appointment.

The scheduling center specialist can also arrange for a telephone consultation with a Carle physician if you prefer.

Our Scheduling Center is available to meet your needs Monday-Friday, 7:30 a.m. to 5 p.m. You or your staff may also initiate this process by faxing the completed Consultation Appointment Request Form to the above number.

Inpatient Direct Admissions or Transfers
An RN will facilitate your patient’s direct admission and/or transfer to Carle Foundation Hospital. The RN will:
  • Coordinate with the house officer for appropriate bed placement for your patient.
  • Contact the appropriate accepting physician to connect with you via a conference call.
  • Stay on the telephone line during the conference call to gather additional information to
  • Facilitate the patient admission/transfer.
  • Accept admission orders to transmit to the hospital unit receiving the patient to expedite care delivery, if the admitting physician prefers this to calling the floor direct.
  • Continue to update you on the status of delayed or more complex admissions/transfers.

Following Your Hospitalized Patients

Carle Foundation Hospital offers referring physicians an on-line way to track their hospitalized patients.  Using the Physician Portal, physicians can login to view current status of their patients and any hospital notes associated with them.
